Custom Orthotics

Introduction to Custom Foot Orthotics

Custom foot orthotics are bespoke devices made specifically for an individual’s foot and condition. Unlike generic insoles, they are tailored exactly to the person’s unique foot structure and needs. Custom foot orthotics can be adapted to fit various types of footwear, accommodate different lifestyles, and address specific presentations. Crafted by hand in an on-site workshop, these orthotics use high-quality materials such as Ethyl Vinyl Acetate (EVA) to ensure durability and effectiveness. Custom foot orthotics are designed to provide support and relief for a range of foot conditions, including flat feet, plantar fasciitis, and knee pain.

What are custom-made orthotics?

Custom orthotics are orthotics which are custom-made, either from a foot impression or a 3D scan. They are form-fitted for every individual’s unique feet to assist in optimising foot posture, pain, and in realigning the body to distribute pressure and gravity evenly.

Most foot conditions and injuries are due to biomechanical abnormalities and abnormal foot position and shape. Issues in the feet can have an impact on the knees, hips, back and spine.

Custom orthotics are used alongside other treatments and therapies to manage conditions that are causing pain and discomfort. Orthotics can be used to treat several lower limb conditions. The design and correction of the device is dependent on the condition and are used as a long-term treatment option.

Podiatrists play a crucial role in prescribing orthotics based on individual assessments, ensuring that each patient receives the most effective treatment for their specific condition.

At Foot Lab Podiatry in Prestons, our experienced podiatrists use comprehensive biomechanical assessments and radiological imaging, if needed, to diagnose conditions such as:

  • Achilles tendinitis
  • Bunions
  • Morton neuroma
  • Plantar fasciitis
  • Heel spur
  • Tibialis posterior tendinopathy
  • Peroneal tendinopathy

Benefits of Foot Orthotics

Foot orthotics offer numerous benefits, including significant pain relief, improved foot function, and enhanced overall foot health. They can help alleviate conditions such as arch pain, heel spurs, and ankle pain. Custom foot orthotics can also improve gait analysis, reducing the risk of further injury or discomfort. By providing support and stability to the feet and ankles, foot orthotics enable individuals to engage in daily activities with greater comfort and confidence. Additionally, custom foot orthotics can be designed to accommodate specific foot types, such as high arches or low arches, ensuring a precise fit and optimal support.

Conditions Treated with Orthotics

Orthotics are used to treat a wide range of foot and ankle conditions, including flat feet, plantar fasciitis, knee pain, and heel pain. They can also be used to alleviate symptoms associated with high arches, ankle sprains, and other foot-related issues. Custom foot orthotics can be prescribed to address specific conditions, such as foot pain, arch pain, or heel spurs. By providing targeted support and relief, orthotics can help individuals manage their symptoms and improve their overall foot health. Melbourne podiatrists often recommend custom foot orthotics as a treatment option for various foot and ankle conditions.

How do orthotics work?

By optimising foot posture and position, orthotics help normalise muscle activity, ensuring that muscles do not have to overwork to compensate for the malfunction. Muscles and joints will function as they should. The support and cushioning from the orthotic will provide comfort and alleviate pressure coming from the ground to the foot (ground reaction force).

The design of the orthotic according to the condition and foot type can alter the mechanics of the foot and increase or reduce the pronation/supination moment of gait. Excessive pronation or supination forces can result in injury. The heel cup is crucial in providing comfort and reducing pressure on the foot, as the choice of materials can significantly impact individual needs.

If a patient has a structural abnormality such as a leg length difference, the shorter side can incorporate a lift which can correct the difference, preventing potential problems to the spine, pelvis, knees and feet.

Process

At Foot Lab Podiatry, a thorough examination is conducted to ensure the custom orthotic prescribed suits the needs of the patient and provides optimal support. The following are conducted by our experienced podiatrists at Preston’s clinic to develop your custom-made orthotic.

Biomechanical assessment

The assessment includes a full history taking, a physical assessment of the patient’s hips, knees, ankles and feet. The strength and function of the muscles, joints and ligaments of the lower limb are tested to accurately diagnose the condition, the causative factors, and any muscle weaknesses. A visual gait assessment is also conducted to identify any imbalances and walking deformities.

  • Further investigation or imaging referrals, if required, such as x-ray or ultrasound to confirm diagnosis
  • 3D scanning or moulding of the foot impression
  • Orthotic designed
  • Prescription form filled and submitted according to the assessment findings, symptoms, occupation and footwear
  • Highly trained and experienced laboratory technicians manufacture the custom orthotics with the highest standard and quality materials

Orthotic therapy plays a crucial role in treating foot and ankle conditions by altering ground reaction forces during movement to alleviate pain.

Orthotic review/follow-up appointment

  • 6 weeks post-orthotic fitting, your podiatrist will book you a complimentary orthotic review appointment to ensure the orthotics are targeting your presenting condition or if any adjustments are required. It is highly recommended to attend the orthotic review appointment to ensure optimum outcomes.
  • Within the first couple of weeks, the orthotics should be worn in gradually, and some pain and discomfort may be experienced. Once the wear-in phase has passed, the orthotics should not cause any pain or discomfort.
  • Any adjustments or modifications required during the orthotic review appointment phase are free of charge. Once the orthotics are positively targeting your condition and causing no issues, it means the prescription is successful. If the orthotics become an issue later on in time it most likely indicates you have outgrown them, or you may require a new updated pair.

Frequently Asked Questions

What makes custom orthotics different to the insoles you buy at the pharmacy?

Prescription custom orthotics are specifically designed to treat your individual foot problem and provide superior support compared to cheaper versions of arch supports. They can be manipulated to control the function of the foot based on the findings of a professional biomechanical assessment. The height of the custom orthotic can be modified, and the rearfoot, midfoot and forefoot can be corrected differently based on the presenting complaint and findings. Additionally, custom orthotics come with specific features that enhance their effectiveness by targeting support to improve pain relief and foot function. The function, materials quality and durability of the custom orthotics are superior to those of the shelf insoles. Adult custom orthotics can last for years and can easily be modified and renewed in our labs.

How Custom Orthotics are Made?

The custom orthotic process involves a thorough assessment of the individual’s foot and ankle, including a biomechanical assessment and gait analysis. This information is used to create a customised orthotic device that meets the individual’s specific needs. The orthotic is then made using high-quality materials, such as EVA, and is designed to provide optimal support and relief. The custom-made orthotics are crafted to fit precisely into the individual’s shoes, ensuring a comfortable and supportive fit. The whole process, from assessment to fabrication, is typically completed within a few weeks, allowing individuals to quickly experience the benefits of custom foot orthotics.

What Materials are Used in Custom Orthotics?

Custom orthotics can be made from a variety of materials, including EVA, polyethene foam, and graphite. The choice of material depends on the individual’s specific needs and the type of orthotic device being created. For example, EVA is often used for its shock-absorbing properties, while polyethene foam is used for its durability and support. Graphite is also used in some orthotic devices for its lightweight and flexible properties. The materials used in custom orthotics are designed to provide optimal support, comfort, and relief, and are often combined to create a customised orthotic solution. Melbourne podiatrists work with individuals to determine the best materials for their custom foot orthotics, ensuring a precise fit and optimal support.

Does everyone need custom orthotics?

No, after a full, comprehensive biomechanical assessment is conducted, manual therapy and conservative treatments are used, our podiatrists will determine whether you will need orthotics. Custom orthotics are only prescribed to patients when necessary, and it is important to wear appropriate casual shoes with orthotics to ensure maximum benefit.

Additionally, custom orthotics may be required for other shoes, like dress shoes, which often have removable inserts. This highlights the need for multiple pairs of orthotics to accommodate various shoe styles, ensuring optimal foot health and comfort.

Are custom orthotics a good investment?

Yes, custom orthotics are personalised orthotic solutions that reduce the need for muscle compensation, thus reducing pain and the need to seek alternative pain relief treatments that can be costly over the long run.

Additionally, purchasing a second pair of custom orthotics can offer financial benefits, such as discounts, and provide convenience for different types of shoes.

How long does it take to get my custom orthotics?

Orthotics are prescribed by podiatrists and issued about 2 weeks from casting. In some cases, we can have them ready sooner at an additional cost.

Orthotics can significantly improve the ability to walk comfortably by providing the necessary support and alignment.

How long do custom orthotics last?

If orthotic care instructions are followed, and wearing orthotics as instructed is maintained, orthotics can last a long time. If the orthotic top cover is damaged or worn out, we can easily recover. However, if the orthotic shell is damaged or cracked, a new orthotics need to be made. Over time, the density of the material can also be compromised and needs to be replaced. The particular activity, frequency of use and the number of times the orthotics are moved from shoe to shoe all impact how long the orthotics last.

The main types of materials used in custom orthotics include cushioning types and support materials. These materials are crucial for ensuring both rigidity and comfort, which are essential for effective and comfortable orthotics tailored to individual needs.

Does a private health fund cover part of the orthotic cost?

If orthotic coverage is available on the policy, 70-80% of the orthotic cost can be covered. Many patients benefit from orthotics, experiencing significant improvements in comfort and mobility.

Orthotics are particularly effective in managing lower extremity issues, such as injuries related to the legs and feet. Scientific research supports the role of these insoles in treating and preventing such conditions, making them a valuable option compared to prefabricated options and custom orthotics.

Your feet are your foundation for life.
