What are custom made orthotics?
Custom orthotics are orthotics which are prescription made either from a foot impression or a 3D scan. They are form-fitted for every individual unique to their feet to assist in optimising foot posture, pain and assist in realigning the body to distribute pressure and gravity evenly.
Most foot conditions and injuries are due to biomechanical abnormalities and abnormal foot position and shape. Issues in the feet can have an impact on the knees, hips, back and spine.
Custom orthotics are used alongside other treatments and therapies to manage conditions that are causing pain and discomfort. Orthotics can be used to treat several lower limb conditions. The design and correction of the device is dependent on the condition and are used as a long term treatment option.

How do orthotics work?
By optimising foot posture and position, muscles do not have to overwork to compensate for the malfunction. Muscles and joints will function as they should. The support and cushioning from the orthotic will provide comfort and alter pressure coming from the ground to the foot (ground reaction force).
The design of the orthotic according to the condition and foot type can alter the mechanics of the foot and increase or reduce pronation/supination moment of gait. Excessive pronatory
or supination forces can result in injury.
If a patient has a structural abnormality such as a leg length difference, the shorter side can incorporate a lift which can correct the difference preventing potential problems to the spine, pelvis, knees and feet.
Process
At Foot Lab Podiatry, a number of steps are taken to ensure the custom orthotic prescribed suits the needs of the patient and provides optimal support. The following are conducted by our experienced podiatrists at Prestons clinic to develop your custom made orthotic.
- Biomechanical assessment
The assessment includes, a full history taking, a physical assessment of the patient’s hips, knees, ankles and feet. The strength and function of the muscles, joints and ligaments of the lower limb are tested to accurately diagnose the condition, the causative factors, and any muscle weaknesses. A visual gait assessment is also conducted to identify any imbalances and walking deformities.
- Further investigation or imaging referrals, if required, such as x-ray or ultrasound to confirm diagnosis
- 3D scanning or moulding foot impression
- Orthotic designed
- Prescription form filled and submitted according to the assessment findings, symptoms, occupation and footwear
- Highly trained and experienced laboratory technicians manufacture the custom orthotics with the highest standard and quality material
Orthotic review/follow-up appointment
- 6 weeks post orthotic fitting, your podiatrist will book you a complimentary orthotic review appointment to ensure the orthotics are targeting your presenting condition or if any adjustments are required. It is highly recommended to attend the orthotic review appointment to ensure optimum outcomes.
- Within the first couple of weeks, the orthotics should be worn in gradually and some pain and discomfort may be experienced. Once the wear in phase has passed the orthotics should not cause any pain or discomfort.
- Any adjustments or modifications required during the orthotic review appointment phase are free of charge. Once the orthotics are positively targeting your condition and causing no issues, it means the prescription is successful. If the orthotics become an issue later on in time it most likely indicates you have out grown them or you may require a new updated pair.

What makes custom orthotics different to the insoles you buy at the pharmacy?
Prescription custom orthotics are specifically designed to treat your individual foot problem and can be manipulated to control the function of the foot based on the findings of a professional biomechanical assessment. The height of the custom orthotic can be modified and the rearfoot, midfoot and forefoot can be corrected differently based on the presenting complaint and findings. The function, materials quality and durability of the custom orthotics is superior to the of the shelf insoles. Adult custom orthotics can last for years and can easily be modified and renewed in our labs.
Does everyone need custom orthotics?
No, after a full comprehensive biomechanical assessment is conducted, manual therapy and conservative treatments are used, our podiatrists will determine whether you will need orthotics. Custom orthotics are only prescribed to patients when necessary.
Are custom orthotics a good investment?
Yes, custom orthotics reduce the need for muscle compensation, thus reducing pain and the need to seek alternative pain relief treatments that can be costly over the long run.
How long does it take to get my custom orthotics?
Orthotics are issued about 2 weeks from casting. In some cases, we can have them ready sooner at an additional cost.
How long do custom orthotics last?
If orthotic care instructions are followed, orthotics can last a long time. If the orthotic top cover is damaged or worn out, we can easily recover. However, if the orthotic shell is damaged or cracked, a new orthotics need to be made. Over time, the density of the material can also be compromised and needs to be replaced. The particular activity, frequency of the use and the number of times the orthotics are moved from shoe to shoe all impact how long the orthotics last.
Does private health fund cover part of the orthotic cost?
If orthotic cover is available on the policy, 70-80% of the orthotic cost can be covered
